CHILD CARE AGREEMENT

Between Shoberry’s Daycare Centres

The terms of this agreement are as follows:

is due on a bi-weekly basis. First fee is due on or before the first day of care and subsequently on or before the payment due date.
  • Shoberry’s Daycare Centre requires a current copy of your child’s immunization records prior to his/her start date. Please also note that all information provided to us needs to be filled out in full. This includes names and addresses including postal codes as well as doctor’s information. Failure to provide us with this information will result in your child being unable to start at Shoberry’s Daycare until the proper documentation has been provided.
  • The fee is payable whether or not the child attends care on the above specified days. The caregiver will supply the following items: Diapers, wipes, a water cup with a lid, formula, special diet foods and an appropriate change of clothing. If the daycare uses their supply you need to replace them. The daycare provides bed linens that are laundered weekly. We ask that the caregiver provide a blanket for rest time. It is the responsibility of the caregiver to launder the child’s blanket weekly. The daycare Centre will provide breakfast, snacks and lunch. Caregivers of children aged 6-12 need to provide a packed lunch as per the bagged lunch policy of Shoberry’s Daycare. Caregivers neglecting to send a lunch will be charged $5.00 per lunch and a lunch will be provided. It is a requirement that a reusable drinking vessel be brought to the daycare that can be kept here for outdoor time.
  • It is the parents responsibility to read the Parent Handbook, acknowledging and respecting the policies and procedures set out within. Any questions regarding the Parent Handbook should be taken to the direct supervisor of that centre. The parent handbook can be found on the Shoberry’s website. In the event of any changes to the policies and procedures, the parents shall be notified via email.
  • For the termination of care, both parties agree to submit in writing two weeks’ notice.
  • Shoberry’s Daycare Centers reserve the right to terminate care at their discretion if in their opinion the child is a hazard to the safety or rights of others or who appears to have rejected the reasonable expectations of the daycare.
  • Written notification will be given by the caregiver for the vacation periods and for extended absent days.
  • Written notification will be given by the Centre for any increase in child care fees and unavoidable or unscheduled closing unless by emergency.
  • Shoberry’s Daycare is not obligated to hold a child’s spot after fifteen absentee days by the child.
